Usefulness of Computed Tomography Perfusion in Treatment of an Acute Stroke Patient with Unknown Time of Symptom Onset

چکیده
A 43-year-old Caucasian male with history of mechanical mitral valve was found down outside a recreational park shortly after daybreak. Examination on arrival to the emergency department revealed altered mental status, right hemiplegia, forced leftward gaze, and complete aphasia. Patient was ineligible for tissue plasminogen (TPA) therapy due to unknown time of symptom onset. Computed University of Tennessee College of Medicine Chattanooga, Chattanooga, Tennessee

Symptom-to-needle Times in Acute Ischemic Stroke and Its Prehospital Related Factors
Background: Intravenous (IV) recombinant tissue Plasminogen Activator (rtPA) (IV-rtPA) is the only FDA-approved pharmacological therapy for treatment in acute ischemic stroke and the administration of IV-rtPA is crucially time-dependent. Objectives: This study aimed to evaluate symptom-to-needle time and factors associated with the prehospital delay in patients with acute ischemic stroke refer...
متن کاملReliability of Persian Early Computed Tomography Score in Patients with Brain Infarction
Background: The one-third middle cerebral artery (1/3 MCA) method and Alberta Stroke Program Early Computed Tomography Score (ASPECTS) were used to detect significant early ischemic changes on brain computed tomography (CT) of patients with acute stroke. We designed the Persian Early CT Score (PECTS) and compared it with the above systems. Methods: The tomograms were chosen from the stroke data...
متن کاملWake-up stroke: clinical and neuroimaging characteristics.
BACKGROUND Approximately 25% of ischemic stroke patients awaken with neurological deficits. In these patients, in whom the time from symptom onset is uncertain, brain imaging is a potential strategy to characterize the ischemia duration and the presence of salvageable brain tissue. METHODS We prospectively evaluated consecutive patients with acute ischemic stroke. CT angiography and CT perfus...
متن کاملPrevalence of Imaging Biomarkers to Guide the Planning of Acute Stroke Reperfusion Trials.
BACKGROUND AND PURPOSE Imaging biomarkers are increasingly used as selection criteria for stroke clinical trials. The goal of our study was to determine the prevalence of commonly studied imaging biomarkers in different time windows after acute ischemic stroke onset to better facilitate the design of stroke clinical trials using such biomarkers for patient selection. METHODS This retrospectiv...
متن کاملThe Acute STroke Registry and Analysis of Lausanne (ASTRAL): design and baseline analysis of an ischemic stroke registry including acute multimodal imaging.
BACKGROUND AND PURPOSE Stroke registries are valuable tools for obtaining information about stroke epidemiology and management. The Acute STroke Registry and Analysis of Lausanne (ASTRAL) prospectively collects epidemiological, clinical, laboratory and multimodal brain imaging data of acute ischemic stroke patients in the Centre Hospitalier Universitaire Vaudois (CHUV). Here, we provide design ...
